Your Request Form is included in the document wallet attached to the outer of your test kit box. It’s a Selph branded piece of paper with your unique kit ID number on it.
You’ll need to fill out the request form with your personal details and the sample date and time and send it along with your sample return - it's this piece of paper that lets the laboratory know which clinic, for example Selph, is requesting the test for you.
If you can’t locate your request form please get in touch with us before sending your test sample to the lab.
