Our request form reminds you to write your sample date on your sample label. However, not every test requires this, and some just ask you to write name, date of birth and sample type on the sample tube label. If you have one of these tests, just follow the instructions and don't worry about writing sample date on there as well.
Please remember to write you sample date and time on your paper form, and include it when you activate your test online.
